The Mental Health Practitioner provides counseling services including assessment, diagnosis, and treatment for patients experiencing social/behavioral/mental health issues. This position does not directly manage any other caregivers. Essential functions include facilitating patient and family learning, performing patient assessments of social, economic, emotional, and family situations, providing diagnostic, preventative and therapeutic treatment services of a psychosocial nature, respecting patient privacy and confidentiality, functioning as a member of the Interdisciplinary Care Team, providing individual, group and family-based therapies using evidence-based treatment modalities such as DBT, ACT, CBT, Solution-Focused, Motivational Interviewing, working in a fast-paced environment, completing psychosocial, economic, and risk assessments and treatment plans, assessing potential threats of harm, providing crisis intervention, psychoeducation and case management support, maintaining knowledge of state and federal regulations, coordinating after-care with outpatient providers, attending unit meetings and trainings, creating a healing environment, documenting patient care, providing therapeutic presence, supporting Lean principles, maintaining a safe environment, conducting activities with professionalism and confidentiality, delivering customer service and patient care promoting goodwill, and performing additional duties as assigned. Additional duties specific to PAT/PES Unit include conducting crisis, risk, and psychosocial assessments, treatment planning, daily rounding of PES and medical unit patients, follow-up risk assessment and safety planning, and coordinating psychiatric inpatient admissions. Additional duties specific to Sage View include working with attending psychiatrist to develop diagnosis and treatment goals and leading multidisciplinary treatment team meetings. Education required is a Master’s of Social Work or related behavioral health field. Licensure required is current unrestricted Oregon LCSW, LMFT, or LPC licensure or ability to obtain state board licensure as a Registered Associate within 3 years. Code Grey Training and AHA Basic Life Support certification required. Experience required is one year in behavioral health field. Physical requirements and PPE use are specified. The position is part-time with variable shifts and 30 weekly hours.
