Job Description

Job Overview:

We are seeking a compassionate and dedicated Case Manager to join our team. The Case Manager will be responsible for providing support and advocacy to individuals and families in need, helping them access resources, navigate services, and achieve their personal goals. The ideal candidate will have a strong commitment to social services, excellent communication skills, and the ability to work collaboratively in a fast-paced, team-oriented environment. A clear driving record is required for this position, as the Case Manager will need to travel to client locations as necessary.

Key Responsibilities:

  • Conduct initial client assessments to determine needs, goals, and eligibility for services.
  • Develop and implement personalized case plans to help clients achieve their goals and improve their well-being.
  • Provide advocacy and support in accessing community resources, including housing, employment, healthcare, and financial assistance.
  • Monitor and document client progress, ensuring that case files are maintained in a timely and accurate manner.
  • Coordinate services with other community organizations, social service agencies, and healthcare providers.
  • Conduct regular follow-up meetings with clients to assess their progress and adjust case plans as needed.
  • Provide crisis intervention and support to clients in emergency situations.
  • Maintain confidentiality and adhere to ethical standards of the organization.
  • Assist clients with paperwork, applications, and other necessary documentation for services and benefits.
  • Participate in team meetings, case reviews, and training sessions to stay updated on best practices and community resources.
  • Maintain a clear driving record and be available for transportation needs related to client services, including visits to clients' homes and appointments.

Qualifications:

  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field (preferred).
  • Minimum of 1-2 years of experience in case management or a related field.
  • Knowledge of community resources and services.
  • Strong organizational and time management skills, with the ability to handle multiple cases simultaneously.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently as well as part of a team.
  • Must possess a clear driving record and be able to use personal transportation for client-related duties.
  • Experience working with diverse populations and in a non-profit environment is a plus.
  • Knowledge of confidentiality laws, social service policies, and ethical guidelines.
